I imagine this requires a triangle formation. One guy from the front. The two guys in back must enter with about a 120 degree angle of separation between them. Sorta like the Tri-Force of Anal.

And at those angles, none of them can actually "get in there", so it really generally ends up as 3 guys awkwardly rubbing themselves against each other, unable to fit.
A "triple" penetration with one of the "dicks" being a dildo would seem to work better, if getting three things in there at once is the aim. That would be the most reasonable (as if this is reasonable? I don't know.) way to go.
Besides, when there's all those guys, there is no good angle on the penetration anyway, making it more pointless.
And the clumsiness of everyone trying to fit at once ruins any momentum a scene might have, too.
So there. That's my complete thoughts on the subject for now.
