I've also wondered how much it is staged - definitely sometimes stuff that stuff from that group of sites does draw blood though - I guess I just struggle to believe that if the pain they inflict is really so severe that they still have people queuing up to appear in their films for relatively small sums of money (if the amounts mentioned on Cards of Pain, Wheel of Pain etc are to be believed). I get how they might find people down on their luck - but there are still some established porn stars who end up there - who don;t show much sign of being into this type of stuff in the rest of what they do.

The episode of Cards of Pain with Luna Corazon suggests that some of their performers really don't know what they are getting themselves in for (notably it is also in English - so there is no chance for the translation to be tweaked).
