An example would be (one of my favorite stars from yesteryear) Raven's "first" anal scene in Anal Encounters #1 (I think). It was with Randy West, if I remember correctly. The lead up to the actual anal is good, but the shot for the actual anal is horrendous. Raven's sitting in a chair, with her ass hanging a little over the edge, and Randy takes her ass from that position (basically missionary). The problem is the camera goes to this underneath shot so you can "see the action up close." The problem is the sound gets muffled, and you can't see Raven's face at all. You don't see it again until he pops. Raven did a number of other anals (foreign films if I remember right) though, so I do believe it was really her taking it. Just a bad director decision. Had high hopes going in, but was VERY disappointed. [Link]

That was a letdown but less of a letdown overall because she's not usually a scorching hot performer like Gianna (for example). Incidentally, isn't Rachel Starr actually a Latina?
