Hormones are a trial and error thing. They can spend a couple years changing dosage and types based on the body's reactions. It takes a while to get it right and sometimes they don't ever quite get it right, just one of the struggles of being trans.

In an ideal world they can get on hormones before puberty and that can make them not need much after that as they essentially can "miss puberty". In some cultures its pretty ok to transition young (like in Brazil or Columbia). Here in North America we still have a ton of religious and right political types that are heavily opposed to kids transitioning at 10 or 11 years old. And most trans people in North America are not "out" until their late teens or 20s because of parents ,churches, etc. so they dont have the chance. I recently dated a girl who didn't start transitioning until almost 30 because of religious and family pressures. She went through hell for her whole life trying to hide she was a woman. We are still so far behind on trans equality in North America.
