The second part of your question was this:
"If they have to, and just got filmed and they refuse to sign the release, what happens next?"
Signing the release means that they are of legal age and agree to perform certain acts and that they release the film rights to the director/studio to distribute as they wish.
As Bill said, that paperwork is done before the cameras roll.
Then that director/producer will have a hard time finding performers for his/her future projects. Performers are almost always represented by an agent who represents many performers. If a director/producer screws one of an agents clients, it is unlikely that he/she will ever be able to hire another of that agents clients again. The porn business is a small world. Once one agent's client gets screwed, word will spread fast about the person who is doing the screwing.
