Actually, it's usually just the fingers and the thumb bunched together (think of extending your hand for a handshake then collapsing your fingers and into a compact line). And the whole package usually only goes as far as the knuckles. It's actually not that big, especially with women's hands--sometimes smaller than typical dildoes and other toys. The asthetics are important too...I just saw Crazed with Serenity, and she has a fisting scene with Missy (well, sort of--they use a lifelike hand toy instead of the real thing, but you get the full effect anyway).
