I recall a scene between Jack Napier and Shyla Syles when this happened. I think it is from There's Something About Jack #22(?). There's a part in the scene when they are in the missionary position and Shyla has her legs in the air above her head. During the action, one of her shoes falls off and hits her on the head. It kinda looked like it had to hurt, but the action continued uninterrupted. It was as if she didn't even feel it. She was obviously too focused on the sex.
