I think a big part of it has to do with the cost/benefit analysis that producers/stars consider when deciding what projects to do.

Men are aroused by different things than women (for the most part). Therefore, even when you have stars such as Sandra Romain or Sierra Sinn who are obviously into what they're doing, its a big leap from doing a gangbang with 6 guys to doing one with 20-50. Their in it for the money as much as sex, so if you can get off get get paid well for doing 6 guys, why take on the added risk of doing more.
