Are we talking camera man or tripod? If tripod you want multiple angles.
You need to be really organised without a camera man.
Also it's probably advisable to ensure the talent is well prepared before hand. And best they don't move around a lot, like bending over/down when they puke.
Shooting puke perfectly can be very difficult without a camera man, and a well organised process.
