My wife's friend suggested I was submissive based on my public persona (reserved, considerate, non-threatening).
But in the bedroom I take a more dominant role.
Which situation determines whether I'm sub or dom? Is everyone naturally one or the other?

I think it's a little of both and in various degrees.
I don't think being quiet and reserved in public makes you a sub. For all I know you may turn your wife into your personal cum receptacle when you two are alone.
My girlfriend is pretty outgoing in public, but in bed she likes me to take charge. She likes to be told what to do and she loves it when I make her beg for cock.
You'd never know it to look at her in public. She's actually quite a little ball breaker out of the bedroom.

Honestly I don't see the appeal of being dominant outside of a sexual context. I'm not knocking it, I just don't understand the appeal.

^ I do not. There are many people that are dominate (or submissive)in every other aspect of their daily lives and yet in the bedroom they will either embrace it or surrender it. Either way, what it all comes down to is giving in to a need. It can be either role play or have it's own dynamic depedning on the idividuals involved.
If you said to me that D/s was just role play, I might shrug it off or even agree with certain points about the statement. I'm not into D/s, it's not my kink. But try telling that to a Master and slave who've been living D/s 24/7 for the last 20 years. You will definately get a different reaction.

So how can you tell if a person is truly dominant or submissive by nature?
Or is it always a conscious decision or habit to act one way or the other.

Well imo, dominate people are able to get others to do things for them, help them out, etc without being manipulatives assholes. They just naturally generate a certain charisma that allows for that. Someone who is more submissive is more inclined to "serve." They like to be the one helping but they also prefer direction.
Sexually, you just never know until you really talk it out or get into bed with someone.
I had one Dom that swore sideways I was a submissive based on my personal nature. Boy was he wrong.
