The last time I checked, Tawny Roberts and Kelly Kline offered them. However, that was some time ago and I'm not even sure either of their sites are still active.
But what if you want her to do a custom video but doing solo stuff only? That should be a little cheaper minus the overhead of course. For example what if I wanted a custom solo foot-fetish movie, she doesn't even have to take of her clothers, that should come a little cheaper I would imagine.

You got a point there about the name thing, I guess I would only pay that kind of money if the girl would throw in a midget in the scene, that would be my perfect custom vid scenario.
You have to trust the girl though, I mean spending that kind of money and then have to wait for the movie to be made.

Ramsey makes a good point that you have to place some trust in the girl who might (ahem) make your custom vid. Some years ago I actually attempted to order a custom vid and I was completely burned out of US $300.00. (Caveat emptor, indeed). It's not really here nor there as I felt I was gambling at the time, and it's not really all that much money. But it still irks me a bit as it's the principle of the thing. I won't say who the girl is because I have no proof one way or the other, but I will say she is on mrdogcat's list.
Edited to clarify: When I said I was burned out of the money, I meant just that. I didn't mean the vid was not good, because I didn't get a vid at all. After I sent the money, and the girl received it, she stopped answering e-mails from me. I got nothing, but I didn't like it.
I do take a lot of custom orders from my website - mostly for extremely specific fetish requests. Sometimes I will get guys that want POV custom content with a male performer of similiar skin tone with my using their (the viewer's) name throughout the scene. I also offer custom regular boy/ girl or girl/ girl content - where you can specify the US performer that you'd like to see me with ad they type of content you want to see - instead of waiting for a company to pair me with let's say, Hush Hush's contract performer Richard Mann for an anal scene, you can book it yourself. Rates vary depending on if there is another performer in the scene and the rate for that person. I include links to most of the talent agencies for you to choose my co-stars as well!
