Quite a few people have consistently pointed out that he is bad news, cheating on her, and a narcissist (which is likely true, given his history and behaviour). So, I think she (with his help) came up with the story about the brother and nephew to avoid people criticizing her when she talks about him and what's generally going on in her life. For instance, when she visits him in Las Vegas for at least a week every month (he never visits her), or when she helps him get custody of his child, etc.
I don't necessarily think she's a bad person. She seems very genuine. She just hasn't thought through the fact this story makes it easier to get people to donate when she refers to them as her brother and nephew instead of her boyfriend and his son (which then inadvertently turns it into a sort of scam).
