The real point of your question, IMO, is about acceptance of cosmetic surgery. Many men outside of the porn biz have had many cosmetic surgeries--including business executives, male models, mainstream movie actors, and successful rockers. Which is why I don't agree with those porn fans who are against female performers with breasts implants. These surgeries are just a fact of life. Men are getting pec implants, total body waxing, butt implants, calf implants, eye lifts--and, of course, hair transplants. All in an effort to look better, or enhance their appeal to younger, prettier women (or men). It use to be that men had to go to Europe for many of these surgeries, but they are now very common in the US, and not just among gay men. Just like women, as men get older they want to "improve" their look, in order to stay competitive with younger men. As to the men-in-porn getting bigger dicks question: my belief (based on photos of what surgery-enhanced dicks look like) is there is at least one popular English performer who's dick has been made thicker by a surgeon. But I doubt if he would fess up, and I don't know what good naming names does. I think fans should be more accepting of the fact that cosmetic surgery is a fact of life--both for males and females.
I seem to remember (before invention of viagra) there was an artificial implant device involving a pump. So, why has no porndude capitalized on this approach? Theoretically, one could artificially get a "real" horse-sized cock.
