Little Girls Blue (1978) meets all of your requirements and it's an absolute classic. In fact, if you haven't seen it already, it will probably be your dream movie. It almost seemed like you were describing the plot. I believe the uncut version is still available on DVD from VCX in the US. There was an excellent PAL DVD from Wildside (sourced from film) which now appears to be out-of-print. There's also a sequel very much along the the same lines.
Me too, I bought seven of their releases at the time and they're great. I really liked the effort they put into their menu designs and interview segments, which were far more professionally shot than I'm used to seeing from the smaller labels. Better than many mainstream studios in fact.
