Usually the only time this happens is when a studio shoots a hardcore porn movie (with pornstars) that has enough plot/dialogs to try and also sell a heavily censored version of it to a late-night tv channel, but they still distribute the hardcore/uncensored version on the usual porn channels/stores/websites. In actual softcore movies the actresses typically wear a pussy patch/bandaid and the actors wear a cock sock, all to prevent genital contact. There have been some rare cases where the softcore actors were so into each other that they went the "extra mile" and actually had sex in the scene, but the angles/censorship mean the viewer doesn't really get to see it(and the actresses wouldn't want it to be shown). It's only recently with 1080p bluray re-releases of these movies that we get to see a few more details than the directors and actresses intended.
