That's why I'm very selective about which pornstars I watch. First, I weed out every woman who has had anal in porn (that right there probably eliminates much of the questionable shit). Next, I remove any woman who has a clearly traumatized butthole and/or often hides it. I've also been going for women who are 2+ years gone of late, so as to avoid disappointments.
