(I had to look up what cuckold means; it basically is a form of s/m where the wife cheats on the guy, the guy knows about it and he's not allowed to do the same.) It's just like with Seinfeld when Kramer bursts into the room, bumps his head hard and you see that Elaine holds in her laughter but continues the scene. This might just be a one movie feat, I don't know. I remember this other scene where two blondes give a blowjob, the penis comes out of her mouth, hits her forehead and they both laugh.
