The combo you have in the picture consists of two uni-polar electrodes. The one with the egg-like end goes inside the anus and sits on the prostate. The other one is worn over the head of the cock. When you turn on the juice, a current (which can vary in terms of both intensity and wave pattern) passes between the two uni-polar electrodes in a circuit -- i.e., down from the head of the cock to the prostate and back.
