When you have too much of anything it can tend to make you throw up. I can't remember the title of the movie but apparently Maggie Star swallows a ridiculous amount and there is footage of her throwing up after.

You would need to ask the girl involved. i can't speak for all women. I'm sure there are pornstars out there who hate certain acts but do them just to get paid, whereas there are others who genuinely like what they do. You can tell by a girls facial expressions and reactions. I try to steer away from porn where it's obvious those invloved are not enjoying what they're doing, because it's not very erotic. Anal and DP's are not painful if the men know how to treat the women correctly and have sufficient lubrication. Everybody enjoys different sexual acts and what some may find awful others may love. The only may to find out is to do it. AS far as doing drugs/ alcohol go I think it'd be kind of hard to do a decent scene drunk or stoned, as you'd know yourself if you've had sex under the influence. I'm sure some girls take drugs but this is also a job and there are many other girls who can easily replace those who are always turning up to work stoned and be much more reliable.

As far as I know all perfomers need to show there HIV tests before they can perform in a scene and they need to do them monthly I think. As far as STD's go some are really hard to detect so I'm not sure on that one - perhaps some industrty performers can shed some light. However if you are a well informed porn star you will have an undersatdning of STD's and be able to check out the people who you will be having sex with to see if they have any symptons.
