Just for kicks. Let's do some quick calculations. I'll use Adriano as until recently he produced and starred in his own movies so technically speaking he didn't have to pay himself a b/g rate. He releases one new scene each 5 days so that comes out to 73 scenes per year. I'm using Aurora Snow's recent article on performer rates so Mike spends a minimum of 65,700$ each year on basic Vag only porn scenes. I think that amount is significantly higher because most of the performer he hires are either established stars that earn top rates or worse are newbies that cost more because producers want to have their first of anything. I imagine that a non-sex performer only doing PTOGM would get paid at least 400$ so if only a third of the scenes have that in them then that is an additional 9600$.

Would add that the above analysis is US "commercial" porn rates. That doesn't hold outside the US, where porn can be quite a bit cheaper. So, not all bad news.

It may be that, instead of a lack of such scenes being shot, they aren't adequately identified. You'll see websites identify a scene as "anal" or "facial," but "p2ogm" is a rarity, and there's no good shorthand way to describe two of the acts mentioned above. Most of the threesome scenes are, I believe, of the kind I don't particularly care for, and so I long ago began not paying a lot of attention to them. But I'm finding some I wasn't aware of now, and I'm wondering how many I disregarded in the past that held some promise -- and how many would have still been tossed aside.

What I see, though, is a lot of scenes where the girls interact exclusively with each other, or maybe one might go so far as to put her face down near the action and watch and moan a little bit. Or maybe the guy gets his pecs rubbed by one girl while he fucks another.

To me it seems that a lot of P2OGM is something that many girls don't want to do; something that costs more within the context of the scene being filmed, so the producers don't want to invest the extra cash; or something that producers have participants willing to do but believe the market is interested in.

One suggestion is for you to contact one of the sites that regularly produces g/g/b videos and ask them to add a lot more POTGM to their scenes.

Now in regards to size of some threads. The best I can say is that this is one instance where size truly does not matter because there are some threads that have multiple pages and very little content. The most obvious one to me is Mia Melano's thread because it has over 20 pages and it's about a performer who has only five scenes all for the same producer and nothing else.
