Ok, dumb question, but in rough porn or BDSM porn when the girl has a gag in her mouth and/or her arms and legs are tied up, how does she communicate it is getting too rough for her?

I know that at Kink.com, they combine a vigorous head shaking with "Uh-uh, uh-uh" to indicate to stop.
But, to be honest, it's always at least somewhat dangerous to rely on that sort of response as a safeword- it's just nowhere as clear as it ought to be. So there's always some inherent risk, although they do try to minimize it.

The story of Barbara Asher is quite interesting. The cliff notes version? She's a dominatrix, who had a client, Michael Lords, die of a heart attack while strapped to a rack. Instead of calling for help, she has her boyfriend shop the body up and get rid of it.
It's a particularly complicated issue. 'Bad' things can happen during any BDSM session. There's no evidence that Mr. Lords was killed because of the scene. It could have just as easily happened while he was driving to work in the morning (with the added chance of causing a car wreck, killing more people.) Had it happened in his sleep, that would have been the end of the story. Because he was involved in a BDSM session, Ms Asher was put in a lose-lose situation: dispose of the body (and hope she doesn't get caught), or call the cops, with a dead man strapped to the rack, and hope she isn't prosecuted. Until societal views of BDSM change dramatically (say, to on par with the risks we take playing professional sports) this sort of situation is a risk for everyone who picks up a flogger.
Anyway, kink.com's method of safewording is great for shoots. The sort of BDSM play you typically see in a shoot is generally a little tame, compared to what many people practice in their personal lives. Charlotte and I don't specifically use safe words, rather I can usually tell when she's in a good place, and when she's in distress. Whenever a bottom is deprived of the capacity to speak, we generally don't push her hard enough to where safewording becomes necessary, and frequent 'check ins' are necessary. It only takes a moment to lean in and say "are you doing ok?" watch for the head to bob and a muffled "mmmhmmm" (yes) or a shake of the head with an "MMMMMmmmmmMMMmmmm" (no!) For film purposes, that's quickly and easily edited out.

So, you must use Adobe Premiere? lol, thanks for the response my man. Good to know. This whole thing came up due to a couple of hogtied.com scene's I saw where the girls were (or said they were) 100% new to domination and I saw quite a few looks of serious discomfort and a few oww's which caused me to wonder how it worked.

We've (wife and I) been using safe words and gestures for over a decade. The general rule is a word repeated 3 times if not gagged. If gagged and/or bound or otherwise restricted, a gesture of some kind done 3 times in succession. Finger snapping (can be done while cuffed), 3 stamps of a foot, 3 shakes/nod of the head, 3 of just about anything. I know some D/s couples who use props in this manner.....one prop is always held and if it is dropped, the Dom/Domme must replace it immediately. If it isn't accepted, that is sign to stop. This works very well with bit-gags that aren't affixed around the head. The sub must keep their own gag in....this is personally what we use. I won't bind her and gag her simultaneously.
Just remember your safe word kids...."Don't!" and "Stop!" can quickly turn into "Don't Stop!"
