There has to be some feedback from the person who is getting, which is picked up and acted on by the person who is giving. When you are fucking and both of you are into it you get in a grove together and the orgasms happen. I don't see this happening much with oral sex in porn. I can't remember the last time I saw a porn with the guy getting the woman off orally. If the other person is receptive to it I've never had trouble getting someone off with my hands and/or mouth.
