WHAT the F…K is up with the editors of movies, even in current movies?? Who else hates it when chapters start in the middle of a position?? If I want to skip to the BJ, then the beginning of the BJ is where I want to/paid to be!! If I won’t to skip to missionary, I should see initial penetration!!! Editing out the initial penetration and not seeing her welcoming/maybe concerning face is unacceptable? As my Drill Sergeant always said, “It Don’t take NO G.E.D.???”

I think the chapter stops are part of the domain of the disc authorer, not the editor. But I definitely know what you mean.

Yes, that is disc authoring, although sometimes the disc authoring is done by the editor, as is usually the case of small indie companies. I for example edit my movies and do my own authoring at the same time. I think it's a better system that way. I don't know who thinks it is a good idea to start a chapter in the middle of a position, seems pretty careless to me, but if the disc authoring is 3rd party and had no specific instructions, then you can't really blame the disc author either. Blame the studio, lol.

Most editorial programs (Final Cut Studio, Avid, Adobe Premiere Pro) allow chapter markers to be set during the process of cutting, or prior to media export.
The issue is more likely one of time, as delivery schedules in adult are not at all forgiving.

Depends. Sometimes it's the camera operator because they just missed the 'start' when the performers changed positions. They weren't watching and missed the insertion or the 'welcome' look. Sometimes it's the editor because they just didn't think that was important enough to include. This is the part I hate the most. The camera operator usually gets blamed for this, but my experience it's usually the editor. Finally, in the case of the DVD chapter starting late, see above.

Maybe the chapters are set at intervals, like every 5 minutes. I do that sometimes with movies like "Amateur Hall of Fame", where it's more or less a 90 minute continuous scene (and mostly gangbangs, so there's so much stuff going on, I'd have a million markers if I noted them all).
I would bet most editor's author too. So much of the authoring work flow is the same as the editing work flow, it's usually much cheaper to have the editor do it while the footage is still on his system.
