I've seen it only once; it's actually a man dressed in garterbelt/stockings/wig getting a triple fist from 2 girls. The scene is from one of those 1990's (?) German fisting movies featuring the "Dr. Fist" character played by that wacky Robin Williams lookalike guy.

I've also seen what ninja1 is talking about. It's a triple anal fist... the women are tiny and the person being fisted is a shemale. Other person has it right too, although that's triple v. I don't know if it's even possible to adequately prep for triple a. fist but despite what a lot of people may think, prepping and doing double comfortably is 'very' do-able and recovery does indeed occur.
