If it's a contract then that's surely something you need to draw up rather than just use a template. For example royality of images, who owns the images after they've been produced etc. Can the actress use the images for her own gain (advertising and for money?) ... I guess you should seek an example as purely a checklist of the kinds of things you need to think about and for the contract to contain but I would seriously not just copy one without a full review.
Also age checking: Not sure what the exact parallel standard is in the UK but this would be a big definite to protect you legally (or something similar):-

Finally, I guess you could sign up to a popular UK porn site as a fake actress to see what they show as a contract ... not that I condone faking an actress ;)
