It doesn't seem to mean there are messages I haven't read yet because that is the case with folders that are yellow as well.

Blue = new messages since the last time you looked at the thread, as far as I know.

Yes, though that is not 100%. There aren't false positives, but false negatives.

Every time you view a thread the forum software keeps track of it. If you read a thread multiple times it updates the last read time for each viewing. It stored these dates for two weeks (I think, I forget the exact duration. It's a huge table in the database so we have to set a timeout value for data retention.) So sometimes on older threads that you have posted in the folder will be blue even though there are no new posts since you last read the thread.
