If you want to be safe from all communicable disease I strongly suggest you go out and purchase one of those CDC biohazard suits. Herpes may or may not (once again, it's uncertain) be contageous all the time - even if the infected person does not have any visible herpes lesions. The same goes for HPV, it may be contageous all the time, even if the infected person does not have any visible warts - or even after they have been treated. Herpes has been documented to be spread by the hands of infected persons. I haven't found any information on titty fucking infections - I suspect it just hasn't been studied.

Basically, there is risk involved in EVERYTHING. The greatest thing in the world has a lot of drawbacks. Sucks, doesn't it?
To answer your question, there is always a risk of giving or catching something when performing oral sex on a girl. Warts, herpes, etc. It's up to you to find out about the girls history, past tests, etc., then it's up to you to make the decision. Dentaldam is definitely something to consider, especially if you're with a hooker. You'd be nuts to go down on a working girl without it. It may take away from her pleasure (I think, I'm not a girl) and you won't have the same experience, but it does help to prevent the spreading of STD's.
