I had the puffy nipples with no breast development (Tanner stage 2) when I was about 11 years old, but hadn't developed anything else before becoming pregnant at 12. I went from no breasts to lactating without anything in between. My daughter and both my sisters had a distinct "puffy nipples on top of developing breasts" stage, though, so I'll assume that if I had the chance to grow normally I probably would have passed through that as well. Don't feel guilty, not all women complete all Tanner stages even after full physical maturity (of any age). The mammary glands are functional (able to lactate) long before complete breast development and, as anyone with a bad back can tell you, nature doesn't always put any more effort into completing a project than absolutely necessary. As for being pregnant so young, yeah, I wouldn't recommend it. [Link]
