I doubt someone would get HIV during a shoot due to porn companies sexual health regulations, which is getting tested before shooting. Two money shots in one scene doesn't cause HIV transmission. It's common knowledge to porn stars to keep good hygiene, so I don't see how cumming twice without pause can be dangerous. The cost for companies/directors may get pricey, but that's the only risk at that point.
