The best blooper I've seen was on a VHS tape called Boobs, Butts & Bloopers .
It featured Ron Jeremy and some actress whom I never did catch her name. Ron is finishing up from a particularly good blowjob and she is just lapping it up like it was the glaze from a donut. She looks like she's happy as a clam. The very second the director says "cut", she spits it out all over him.
Ron: "Hey! What the hell?" Girl: "What the fuck did you eat? That tastes gross!" Ron: "You spit on my leg!" Girl: "So? You came on my face! No onions next time, huh?"
The whole tape was pretty good. The DVD is NOT the same as the tape, despite the similar titles. Pity.

^I don't know who she is. I wish I did though! :) I have a feeling that's a real blooper. The dude looked too pissed, and the girl too uncomfortable, for it to be fake. I could be wrong of course, but it looked real. I don't really believe the little blurb that accompanies the vid though. That shit about "my buddy edits porn for a living" or whatever... I'm sure that's crap. But the vid looks real. And even if it's fake, it's funny as hell.
I've seen several good ones. Four come to mind that I believe were real: 1. In an amateur/gonzo film from a few years back, some guy new to porn is fucking a woman in missionary on the floor when, without warning, he cums inside of her. The woman looks a little surprised/confused/annoyed, and the director yells, "What the fuck, dude? You're not supposed to cum inside her! You're supposed to cum on her face!" Much laugher ensues. 2. In another scene featuring a newer guy and a porn actress, the guy cums inside the woman accidentally. From off-camera, the porn actress's husband is heard saying, "I want that guy's name and address" (in case, I presume, of pregnancy). Again, much laughter is heard. 3. Some porn guy is standing up as a good-looking porn actress blows him to ejaculation. Her mouth is aligned perfectly as a huge stream of cum shoots from the guy's penis straight into the back of her mouth (and maybe even down her throat), and she immediately gags violently. This was a famous clip on the 'net a few years back and can probably be easily found. 4. In a newer scene, from one of the BangBros-type producers, a man is kneeling on the edge of a bed getting lined up to cum on a woman's face, when all of a sudden he ejaculates over the woman's shoulder and smacks the (close-up) camera guy right in the face. (A second camera from behind camera guy #1 catches it.) Camera guy #1 immediately starts yelling "Cut! Cut!" Hilarious.
I actually love it when they keep the bloopers in the actual scene. I mean I know this won't work in a story movie but for gonzo I think it's pretty cool.
