As you can see my name and date of birth is no problem, as well as zip code, I live in a fairly rural area USA and use PO Box as an address, street, house number and place I think are giving me problems here, attempted a number of different variations but now run into other problems apparently after multiple attempts I've reached the limit of submitting the info, so now I've hit a brick wall so to speak!

Could anyone explain to me what exact information should I place in those boxes, the problem is my American to German translation of address information doesn't work, attempted to contact XJuggler but no response yet.

How would you fill out this form to someone who's in a similar predicament and has made purchases through XJuggler?

On a side note, also subscribed to Jeedoo and have made purchases with absolutely no problem, thanks in advance to anyone that can help.
