Next best is with the guy lying down, and the girl lying on her belly with her legs bent and her feet up in the air. I don't know why, but this angle really does it for me.
Standing can work, but only if the camerawork is excellent - too much room for the BJ being filmed at the wrong angle - e.g. too low, so you can't see the girl's face, or too high, so all you can see is the girl's forehead.
