Actually, positioning 3 guys is possible. Sandra Romain has done a triple A (allegedly as the action is badly caught). Kat Langer did a triple vag that was reasonably well caught. Several gals have done a double v+a or a double a+v showing that the positioning is possible.
The trick is twofold: get a gal who can do a triple and get a cameraperson who is flexible enough to catch the action.
Arianna Jollee allegedly shot a triple A in the Narcissist but I will not count in my books as the scene is NOT in the US version. If it is not in, it does not count for the record books.
Maybe Melissa Laurent might want to try the triple A .

Edited to add: The best position to catch all the action is the CG position. It will be difficult to catch the action on camera with the reverse CG position.

Actually BKP-Janay, there is no conclusive evidence that it was done.
The Sandra Romain scene is not conclusive to the viewer and I am not fully persuaded that it was a triple A. The Arianna scene is a figment of somebody's imagination unless it is released as a DVD in the US. If Anarchy chooses to edit it out of the US version, it does not even exist, much less count.
So the challenge still exist. It must be well shot and released completely in the US.

Unfortunately, that scene was badly shot and the viewer cannot see anything. Somebody should redo a triple anal but get it well lit and shot so the viewer can see what is going on. The two triple vag by Kat Langer were actually reasonably well shot so the viewer knows that it is a genuine triple.
There is still room for more.
According to a review of the Euro version of the Narcassist, there is not triple A so the challenge is still open.
Remember, a good well shot triple A is the goal.

RonK, I have watched that fragment of Sandras gangbang 5 or 5 times in slow motion and it clearly is a triple anal. The thrid dick was not too deep in her ass, though (about 3 cm maximum, if I recall right) and it was a very short episode. It was very badly filmed, though.
