The two that I can think of seemed to have been unplanned:

Funnily enough I watched a scene like that today. In 'Passion of the Ass' scene 1 (Lex Drills Productions DVD; Jenny, Manuel Ferrara and Steve Holmes) Manuel and Steve are double penetrating Jenny. They both mention trying double anal and start to attempt it, then there is a cut and Steve tells Manuel to go in her pussy instead. I can only presume that the attempt did not go too well!

Funny that Avi L is mentioned .

That reminds me of the Charity Love scene in On the Road: South Beach 2. The guys try DPP but the cameraperson pans to show her face rather than down where the action is. They decide she is too tight and that's the end of it. I always wondered if they got both inside or her...or if they even really tried. Other scenes with frustrating camera decisions that come to mind are CreamPie Orgy 12 (the Barbariska scene) and the StudentSexParties videos with Izi Ashley: it seems like the interesting action is always just off camera or obscured by someone's hand, leg or hair.

But I digress...
