My guess is that it happens on occasion but that the idea that it's going to happen anywhere near every time someone goes near an asshole (even a visibly dirty one) is mostly unfounded hysteria. As mentioned, your mouth has secretions that kill bacteria and so does your stomach. It's not foolproof and I wouldn't go so far as to say full on scat would be something that's going to be safe.
