Can someone help me understand the Japanese censorship laws? I've seen adult videos that are censored, lightly censored, or sometimes not censored at all. Why are they all so different?

I haven't been keeping up with censorship law in Japan but the scenes that have no digital fogging I believe are being released outside Japan for foreign audiences so they don't have to follow Japanese censorship requirements.
