By Paula Hall, Psychosexual Therapist and the BBC

Premature or Rapid Ejaculation is believed to affect between 30-40 percent of males. This frustrating condition can a lot of distress for both partners. But the good news is that it's one of the most successfully treated conditions seen by Psychosexual Therapists - and some of the techniques you can teach yourself at home.

Over the years many people have tried to give a definition of premature ejaculation - ranging from 'coming within 6 thrusts' to 'coming within 2 minutes' and even 'coming before your partner'. Of course, none of these are sufficient - particularly the last one if you have a partner that likes to take up to an hour to reach orgasm!

The definition I prefer, is that if you come before you want to and you feel you are not able to control it - you are suffering from premature ejaculation (or PE for short). But, bear in mind that most men will come sooner than they'd like on some occasions, particularly if under stress or in situations of very high excitement.

So, I would go on to say that you only suffer from PE if you are unable to control when you come more than 50% of the time.

Some men may only suffer from PE when they're having intercourse.
Some feel they come too quickly whatever the stimulation with a partner.
Others feel they have little control on self masturbation.

The 'Point of Inevitability'

Sufferers of premature ejaculation aren't able to recognise, what therapists call, the 'Point of Inevitability'. This is a sensation that occurs just a few moments before ejaculation, and men who don't suffer from the condition are able to recognise this sensation and either stop or change stimulation until the urgency has subsided.

Most commonly, men who suffer from PE will have had it throughout all of their sexual life and won't ever have noticed this 'Point of Inevitability'. There are a number of reasons why this might have happened. It may be that initially sex was always rushed or sex was associated with feelings of guilt or anxiety.

Sometimes men become quick ejaculators because of partner pressure to 'get it over and done with'. Whatever the original cause, the body gets used to responding quickly and rapid ejaculation becomes the norm.

Premature ejaculation is rarely a result of a physical condition. However, remember that whenever your body is under stress of any kind, ejaculation tends to be quicker.

Self help techniques

If these self-help techniques don't help - your GP might suggest a short course of antidepressants. The latest style SSRI's have the side affect of delaying ejaculation and are often prescribed purely for that purpose.

Or your GP could give you information about the new Penile Ring - a nifty little devise that has been shown to reduce sensitivity and thereby prolong ejaculation

There are a number of things you can do to help yourself to recognise the 'Point of Inevitability'. You might choose to practice alone at first, then discuss it with your partner and ask them to help you overcome this problem.

Relax - this is the most important thing you can do. Try a relaxing bath, using deep breathing techniques or buy a relaxation tape from your local health shop.(See Get to know your body

Relationship issues - be sure that your head and heart is in the mood for sex as well as your body. If you're unhappy about something with your partner then sort it out first.

Exercise your Pelvic Floor - many men say they are able to delay ejaculation by 'squeezing' or 'pushing' their pelvic floor muscles. (Try our Pelvic Floor Exercises for men).

Stop and Start - this is the most common technique used in psychosexual therapy. Practice stimulating yourself to the point just before ejaculation - then stop. Start again when the sensations have subsided. Repeat this 3 times. Gradually you should find the length of time before each stop is getting longer.

Change strokes - if you feel you've gained more control with the stop and start technique, try changing your stroke to something less stimulating rather than stopping all together.

Change positions - during intercourse, try the stop and start technique when the sensations become strong and when you're more confident, try changing positions rather than stopping.

If none of the self-help techniques work for you, please don't worry. This is one of the most treatable conditions.

Actually, I think that's totally false. I'm sure that almost every man does recognize that point. The problem with those who suffer from PE is that they simply aren't able to prevent that point from arriving too soon. Perhaps it would be more accurate to say that they don't recognize the steps leading up to that point (and/or have enough mental awareness to take appropriate steps / excercises to "back away" from it) - but, even that, may not be universally applicable.

That so-called "Point of Inevitability" is like the event horizon of a black hole: once you cross it, there's no turning back.

Funny, though, that there's no such sexual dysfunction as "pre-mature female orgasming". When women cum fast, it's called "being highly responsive" (although not, perhaps, if they fall asleep right afterward ).

In life as in porn, we men are predisposed (conditioned?) to regard ourselves largely as "tools"; our right to pleasure on our own terms is secondary to our duty to be studs.

Just call me the Two-Minute Man.

I just read that to my husband, and his reaction was to chuckle. He said waiting for me was the best way for both of us to have the best time. He says that (up to a point), extending sex makes for a better orgasm for him. I personally deeply enjoy the feeling of those moments when he holds back the urge to spurge. It definitely makes his orgasm better if he holds back two or three times. That's about ideal for us.

I'm glad I was able to treat Mr. Lindi to a good laugh. Give him my best!

My orgasms are also better when I hold off, and there's nothing more fun and exciting than having Ms. Bob "cum on my cock" (as I like to urge her to do). Still, it strikes me that there's a kind of double standard on this one -- but I guess that would give it roughly the same status as most issues involving men and women.

<damn typos!>

I guess I'm funny, I like the sense of abandon that comes with a fast orgasm. It makes me feel sexy and hot, like he couldn't hold back.

Mind you, not all the time. I typically like "normal" length sessions but occasionally I like him to let loose.

I don't think you're "funny", luna; I think you're sweet. And you should feel hot and sexy . . . because he couldn't hold back.

That IS the cool thing about a good steady relationship; there's lots of time and trust for a wider variety of sexual response and enjoyment.

As someone who has taken SSRIs (though not for PE), I can say they definitely can increase your staying power considerably...

If you mean PE, that would be the topic - Premature Ejaculation.

Effexor is one SSRI, but it also affects something else in the brain, that definitely works, when I was taking 150mg a day I couldn't even masterbate because it took too long, nice for sex though.

That's right, selective seratonin reuptake inhibitors.

I was reading this and trying to decide if it had ever happened to me before. I guess, yes, technically, if you count the times when Mrs. 60 got me so hot and bothered I popped like a champagne bottle in seconds flat. Hey, it happens!

And like lunacy and Bob said, it can be very, very cool -- it makes the missus giggle with pride that she still has that effect on me after 13 years of exclusivity.

I think, though, if it happened all the time or <shudder at the thought> every time, that would certainly be a problem, but not the end of the world. What the hell -- we've got hands, we've got a mouth, and there are toys for that. Intimacy, sharing, mutual pleasuring, that wouldn't and shouldn't have to suffer.

Bob said we're (as men) conditioned to perform -- he's absolutely right. We think we've *GOT* to lay the pipe until our SO has at least one orgasm or we've failed. I know a lot of guys who'll tell you "man, I don't even THINK about cumming until I've made her go off two or three times" and I'm thinking "how limiting, you're treating your partner like a fucking machine, one pop, two pop, three pop -- OK, my turn!!" Am I missing something there or is that spot on? Sometimes I've been told "stop holding back and just CUM DAMMIT!!" I like that.

I'm rambling around a point, I think, about how our perceptions of how sex "should be" gets in the way of how good sex can be (or *IS*) if we just approach it with an open mind. Hangups are hell.

There's so much more to life than the limited set of rules, preconceptions, prejudices, and phobias that get force fed to us from the time we're born.

As I've learned, though, being adept at cunnilingus compensates for nearly any other shortcoming we men may have!

That's spot on, cobalt60. When we men think like that, we're treating OURSELVES like fucking machines . . . and like fucking machines.

Great! Otherwise, I've got some serious selfishness issues to work out...

I wonder if anyone (and this may be a poll) has had issue with the opposite syndrome -- inability to orgasm ("would you get it over with already?") over prolonged periods of pounding? Variable stroke, position changes, sometimes *nothing* works (except forbidden sister-in-law fantasies which is another thread entirely).

Honestly, I think the true scourge of mankind is our prediliction for killing each other in the name of God, but that's also off-topic...nothing more to add, crawling back under my rock.

Cobalt60, the Sex Talk FAQ has some links to seriously interesting member threads on guys who take forever to cum and on trouble cumming with a woman. Neither problem is very uncommon.

Thanks for that, you are a kind and generous soul.

I've read through those and am thankful it's not so extreme in my case. It's very intermittent, nothing I would call a 'problem' and probably has as much to do with being tired or stressed or overindulging the beers as anything else.

Good ol' Beer, or as Shakespeare said in Macbeth:
"Lechery, sir, it provokes and unprovokes; it provokes the desire, but it takes away the performance: therefore much drink may be said to be an equivocator with lechery: it makes him and it mars him; it sets him on and it takes him off; it persuades him and disheartens him; makes him stand to and not stand to; in conclusion, equivocates him in a sleep, and giving him the lie, leaves him."

My experiences with SSRIs: Celexa made me totally asexual. After a month or so I quit and, ironically a lover entered my life for a pre-determined 20 day period. It took 5 days or more for the Celexa to wear off and I could get a full hard-on. Now I am on a generic Wellbutrin (sp?) and I think the sex drive is OK although there is no lover at the moment. Also, earlier in my life I had slight problems with PE but with one girlfriend it totally went away. I had a hard time cumming and she was getting multiple orgasms (10 or more until sometimes she would breakdown sobbing.) I think her orgasms made me feel more powerful so I had more control of my own orgasm.

It's always something,
Percy
