For me, ATM is just something that "happens" during really hot, passionate sex. If your girl is absolutely clean then going from ass to mouth can happen organically. I'm not sure I've ever had a "conversation" about ATM except on set.

There are several diseases, viruses and parasites that wait for just this type of oppurtunity. Not to turn you off to the whole idea or anything, but look at THIS before thinking about atm too much. That means that you can get it from a2m as well. Moral of the story is that the absolute main thing to be sure of is cleanliness. ;) After that, it's more likely to happen naturally than if you discuss while having sex. If you have a particularly strong relationship with your partner, and the two of you are particularly open to new things, I would suggest talking to her about it ahead of time. Administering a quick enema into the lower rectum as opposed to 30 minutes of holding the water is preferred. Release water, squirt again, repeat until clear. This should clean out that area sufficiently for "good clean anal sex"!. And asking during sex is usually the way-to-go.
