Downloading in Onlyfans is not allowed (as per their terms of service), the only way of downloading is if the creator sends you a download link (like Casey Calvert does), or if you use third party tools which aren't allowed. Probably the same tools that are also used to download streams from Netflix, Disney+ etc. You can judge the legality of these yourself.

coz it's not that cut and dry unless you're using browser extensions which would be technically third party tools. Any tech savvy person or someone who knows how to use the inbuilt browser tools will know how to download without these kinda extensions.
So question is, is the browser a third party tool? I wouldn't say so.
Legality of it is that OnlyFans ToS says all their content hosted on their servers is for use only on their servers, but it doesn't do anything from a technical standpoint to mitigate downloading.
