How digital versions of older scenes early 2000 and before mostly look worst than the dvds. There are a couple of well known companies that do this. I give props to Jules Jordan Video for remastering practically its entire back catalog to 1080p. I understand this is costly but more big companies should be doing this.

Scenes where actors stop in the middle of a discussion to look at the camera and address the viewer (breaking the fourth wall). FamilyXXX does this a lot and it looks stupid.
HDV digital cameras were not used until 2003/2004. Before HDV, DV tape cameras were used in the 90's and consumer/broadcast video tape before that. To save costs, most companies recorded the tapes over and over and over and over until they were worn out and thrown away, so footage masters for remastering efforts don't exist for most stuff shot before 2006. Even if there are materials to use, there's not really anything great to be gained from remastering any of the stuff before 2006, because the cameras just couldn't give the level of video quality that you're expecting and the non-pro-grade tape materials would have most likely degraded since they were originally shot. It's also difficult to get stuff off of the older tapes because the older video codecs are not supported by the newer software and functional tape decks (or cameras you can connect via FireWire) for the old formats are also hard to come by.
Shoes on during sex. Why? Out of all the stupid things in porn, a woman wearing nothing but shoes is one of the stupidest. And it's been going on for decades. I'm not a foot fetish guy.
